Terms and Conditions for Massage Therapy Services
1. Service Agreement: By booking an appointment or receiving massage therapy services from our business, you agree to be bound by the following terms and conditions.
2. Client Responsibility: It is your responsibility to provide accurate and complete information about your health condition, medical history, and any allergies or sensitivities you may have. You must inform the massage therapist of any changes to your health status before each session.
3. Confidentiality: Any personal and health information shared with the massage therapist during the session will be kept confidential. The therapist will not disclose any information without your written consent, except as required by law.
4. Consent for Treatment: By booking an appointment, you give your consent to receive massage therapy treatments. You have the right to ask questions, seek clarification, and refuse any treatment or technique suggested by the therapist.
5. Cancellation Policy: If you need to cancel or reschedule an appointment, please provide at least 24 hours' notice. Failure to do so may result in a cancellation fee.
6. Late Arrivals: If you arrive late for your appointment, the session may be shortened to accommodate subsequent appointments. Full payment for the scheduled session will still be required.
7. Payment and Fees: Payment for massage therapy services is due at the time of the appointment. We accept cash, credit cards, or other agreed-upon payment methods. Any additional fees, such as travel expenses or special requests, will be discussed and agreed upon in advance.
8. Health and Safety: The massage therapist reserves the right to refuse treatment if it is determined that it may be harmful to your health or if you exhibit inappropriate behavior. In such cases, full payment will still be required.
9. Liability: The massage therapist will not be held liable for any injury, loss, or damage to personal property that may occur during or after the massage therapy session.
10. Consent for Use of Techniques: The massage therapist may use various techniques during the session, depending on your needs and preferences. By booking an appointment, you give your consent for the use of these techniques, unless otherwise specified.
11. Changes to Terms and Conditions: Our business reserves the right to modify or update these terms and conditions at any time. Any changes will be communicated to clients through our website or other appropriate channels.

Cancellation Policy
Cancelation/ Card Holder Agreement
1.) I agree to having a good standing card on file. In case of No-show appointment and Last minute cancellations ( Information listed below.)
2.) I will give 24 hour notice before a cancelation so that there will be time to fill the appointment. Otherwise I understand the company may charge up to 100% to cover costs of the time unfilled. Exceptions may apply for hospital visits Illness, ECT. I understand that I must communicate that with a front desk personal or Massage Therapist ad Three Peaks Bodyworks LLC. Upon failure to reach out IT will be considered a No-show and be on my client record.
3.) I understand that a No- show appointment is seen as 15 minutes late to the appointment time without any communication. I will be charged the 75% to cover the cost of the time I had.
4.) I understand that if I am late to my appointment that my therapist may take the time off of my massage to accommodate other clients of the facility. IF it is over 15 minutes I may have to reschedule my appointment. This will be up the massage therapist.
